We're a small referral and information service. We are not a community, a care company, a brokerage or an advisor. Our only job is to listen, explain the options clearly, and — if you want — introduce you to independent local businesses that can help. Here's exactly how we work.

In short

Senior Transition Assistance is a free referral and information service for older adults and their families. We help with downsizing and moving, finding senior living, care at home, selling the family home, and understanding veterans benefits. We are run by IMLeverage, LLC, in Lenexa, Kansas. What we do

When a family calls, a person — not a script — asks a few short questions about who needs help and what kind. Then we introduce you to independent local businesses that fit: senior living communities, senior move managers, real estate professionals, and care agencies. They contact you; you decide what to do next. You can also just use the information on this site and never speak to anyone. Both are fine.

What we are not

We want to be clear about this, because it matters when you're making decisions for someone you love. We are not a senior living community, a care provider, a real estate brokerage, or a licensed health, legal or financial advisor. Nothing on this site is medical, legal, tax or financial advice. Costs, program rules and benefit eligibility change and vary by state — always confirm the details with the provider or the government agency itself.

How we're paid

The service is free to you. The partners we introduce — the communities, move managers, agents and care agencies — pay us a referral fee when they serve a family we introduce. That's the whole business model. It never changes the price you're quoted, but you should still shop around and compare, and we'll show you where to check any partner's license and inspection record yourself. If you'd rather we not share your information at all, you can opt out any time; see our privacy policy and do-not-sell page.

Questions about us

Do you charge families anything?

No. Talking to us, using the site, and being introduced to a partner are all free. Partners pay us a referral fee when they serve a family we introduce; you are never charged by us.

Do you recommend one community over another?

We introduce options that fit what you tell us, but we don't rank or rate communities and we don't get to decide what's best for your family. Compare more than one, tour more than once, and check each one's state inspection record — we'll show you where.

Is my information safe?

We collect only what the form asks for — never your Social Security, bank or card numbers, Medicare or Medicaid IDs, or government ID. Your details are shared with the partner who may call you back, and you can ask us to stop sharing or delete your information at any time. See the Privacy Policy.

Are you affiliated with any government agency?

No. We are a private company and we are not affiliated with, endorsed by, or acting on behalf of Medicare, Medicaid, the VA or any state agency. When we point you to a government program, we link you to the program itself.
