# We send families who are ready to talk.

If you run a senior living community, a move-management company, a senior real estate practice or a home-care agency, you know that a family who calls us is usually further along than a lead bought from a list. Here's how the referral works, and what we ask of partners.

In short

Senior Transition Assistance is a free service for families. Families reach us by phone or form; we listen, then introduce them to independent local providers that fit what they told us. The family pays nothing; partners pay us a referral fee when they serve a family we introduce. We're operated by IMLeverage, LLC in Lenexa, Kansas. ## How the referral works

1. **A family reaches us.** Someone — usually an adult child, sometimes the older adult themselves — calls our free line or sends the form, telling us who needs help, what kind, and when.
2. **We listen and match.** We ask a few short questions and introduce providers in the family's area that fit the request: a community at the right care level, a move manager, a senior-focused agent, or a home-care agency.
3. **You reach out.** The family expects a call or message from providers who fit. Their consent to be contacted, including by phone or text, is recorded with the request, and we honor opt-outs and do-not-call rules.
4. **The family decides.** They choose to work with you or not. We never pressure them, and neither should a partner.

## What we ask of partners

- **Hold the license your state requires** and carry current liability insurance, and be willing to show both to a family on request.
- **Answer promptly and honestly.** Tell a family plainly if your setting or service isn't the right fit — that protects everyone, including your reputation.
- **Respect the family's information.** Use it only to follow up on the request you received. Don't resell it or add it to unrelated marketing lists.
- **Follow the disclosure rules.** Where a state regulates senior-living referral agencies, we and our partners follow that state's disclosure requirements, including telling families who pays the fee.

We don't publish a star rating or a "vetted" badge on partners, and we tell families to verify any provider's license and inspection record themselves. That's not us being coy — it's the honest position, and it keeps the relationship based on your service, not our seal.

## Referral fees and the rules around them

We're paid by partners when they serve a family we introduce; families never pay us. There are federal and state rules about referral fees that everyone in this space has to respect. In particular, the federal Anti-Kickback Statute bars paying or receiving remuneration for referrals to items or services payable by a federal health care program such as Medicare or Medicaid — and we don't accept referral fees for those. Most private-pay senior living and transition services sit outside that rule, but any referral that touches a Medicare- or Medicaid-covered service has to be handled carefully. If you're unsure how your service fits, ask.

## How we handle family information

Families trust us with a phone number and a sensitive situation. We keep that data private, store consent records with the request, and don't sell it to unrelated marketers. Partners receive only the details needed to follow up on the request. If you receive a lead from us, handle it the way you'd want your own parent's information handled. ## Questions from providers

### What does it cost to be a partner?

Nothing up front. There's no listing fee or subscription. We're paid a referral fee when we introduce a family and your team serves them. Talk with us about the specific terms for your service.

### How are leads delivered to us?

The family gives their contact details and consent to be contacted, and we pass the relevant details to providers that fit. You follow up directly with the family at the time they indicated.

### Do you guarantee a certain number of referrals?

No. We don't promise volume — no honest referral service can. We send families who match what you provide, when they come to us.

### What kinds of providers do you work with?

Senior living communities (independent, assisted and memory care), senior move managers, real estate professionals who specialize in seniors' homes, and home-care and care agencies. If your service helps older adults move or live well, it's worth a conversation.
